Responsibilities

• Complete thorough needs assessments with adult clients to capture strengths, goals, and environmental factors
• Create and update individualized service plans that outline clear steps, timelines, and measurable outcomes
• Link clients to healthcare, housing, benefits, employment, and social supports available across Louisville
• Monitor ongoing progress through regular follow-up contacts and adjust plans as circumstances change
• Advocate for adults within provider networks, government offices, and community agencies to remove obstacles
• Maintain accurate, timely documentation of all case activities that meets professional and regulatory standards
• Collaborate with multidisciplinary partners to ensure seamless coordination of services centered on the adult client
• Respond to crises or sudden changes by arranging immediate resources while preserving client dignity and choice

What You Bring
