Aequor Allied is hiring a travel Home Health Physical Therapist in Holland, Suffolk City. This full-time position runs 13 weeks at $2,345 per week, working 40 hours per week with 8-hour shifts.

Join Aequor Allied as a travel Physical Therapist specializing in home health care. This temporary assignment provides an excellent opportunity to expand your clinical experience while serving patients in their own environments. The role focuses on direct patient care delivery in the Holland, Suffolk City area, where you'll work autonomously and make meaningful impacts on patient outcomes. Starting July 27, 2026, this 13-week travel therapy placement offers competitive compensation and the flexibility of temporary assignment work.

Responsibilities

• Evaluate patients' physical conditions, medical histories, and mobility limitations to develop individualized treatment plans
• Deliver evidence-based therapeutic interventions including exercises, manual therapy, and functional training within patients' homes
• Educate patients and families on prescribed exercises, mobility techniques, and strategies to improve functional independence
• Monitor patient progress toward established goals and adjust treatment plans accordingly
• Document patient assessments, treatment sessions, and clinical outcomes in compliance with regulatory standards
• Assess home environments for safety hazards and recommend modifications to support patient recovery and independence
• Maintain accurate, timely medical records and communicate effectively with interdisciplinary care teams

Qualifications

• Current Physical Therapist license in good standing
• Current CPR certification (BLS)
• Valid driver's license and reliable transportation for home visits
• Experience or strong willingness to work in home health and community settings
• Ability to work independently and manage patient caseloads with minimal supervision
• Excellent communication and documentation skills
• Strong interpersonal skills and commitment to patient-centered care
