About the Role

RiverWoods Exeter is seeking a dedicated RN/LPN to join our nursing team on the 3p-11p shift. Working 24 hours per week, you'll deliver high-quality, person-centered care to our skilled nursing residents while collaborating with an interdisciplinary care team committed to resident well-being and positive health outcomes.

What You'll Do

• Provide direct nursing care and hands-on support to skilled nursing residents during evening hours
• Assess resident health status, monitor vital signs, and respond to resident needs promptly and professionally
• Collaborate with nursing and support staff to deliver comprehensive, individualized care plans
• Maintain accurate, timely documentation of resident care and health information
• Communicate with residents, families, and team members to ensure coordinated, compassionate care

What We're Looking For

• Current RN (Registered Nurse) or LPN (Licensed Practical Nurse) licensure in New Hampshire
• Experience in skilled nursing, long-term care, or similar healthcare settings
• Strong clinical assessment and care delivery skills
• Compassionate, patient-centered approach to resident care
• Excellent communication and teamwork abilities
• Reliability and commitment to the evening shift schedule
