About the Role
The Clinical Coordinator Behavioral Health Liaison bridges clinical care teams and behavioral health services, ensuring seamless coordination of mental health and substance use support for our diverse patient population. You'll facilitate care integration, manage referrals, and support patients navigating behavioral health treatment alongside their primary care.
What You'll Do
- Coordinate behavioral health appointments, scheduling, and patient follow-up to improve treatment engagement
- Process referrals between primary care providers and behavioral health specialists
- Maintain accurate medical records and documentation related to behavioral health care coordination
- Support patients with insurance verification, authorization requests, and access to treatment resources
- Communicate with clinical teams to ensure behavioral health needs are integrated into overall care plans
- Assist in tracking patient outcomes and coordinating care transitions
- Help remove barriers to behavioral health treatment through patient education and resource connection
What We're Looking For
- High school diploma or equivalent; some college coursework or healthcare experience preferred
- Strong organizational and communication skills with ability to work across multiple departments
- Experience in healthcare settings, patient coordination, or related administrative roles valued
- Compassion for working with diverse populations facing behavioral health challenges
- Proficiency with electronic health records (EHR) systems
- Ability to handle sensitive information with confidentiality and professionalism
