Greenbrier Healthcare Center is seeking a PRN Physical Therapist (PT) in Parma Heights, Cuyahoga County to deliver hands-on rehabilitation care as a valued, in-house member of the CommuniCare therapy team.

This PRN Physical Therapist (PT) position at CommuniCare offers flexible, as-needed scheduling at Greenbrier Healthcare Center in Parma Heights, Ohio. As an in-house team member rather than an outside contractor, you will work directly alongside residents and clinical staff to design and deliver therapy that restores mobility, strength, and independence.

Key Responsibilities

• Evaluate residents and create individualized physical therapy plans of care.
• Provide skilled treatment to improve mobility, function, and quality of life.
• Track progress, adjust interventions, and document outcomes accurately.
• Coordinate with the interdisciplinary rehab team to support each resident's goals.
• Make a direct, meaningful impact on the lives of those in your care.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, valid licensure to practice as a Physical Therapist (PT) in the state of Ohio.
• Commitment to compassionate, resident-centered rehabilitation care.
• Availability to work on a PRN (as-needed) basis at the Parma Heights location.
