About the Role

As the Manager, Care Management at Humana, you will oversee and develop high-performing teams of nurses, social workers, behavioral health clinicians, and care management support staff. Based in Stateville, Will County, this position centers on coordinating quality care for Medicaid members throughout Humana's Illinois market, ensuring members receive timely, compassionate, and well-organized support.

Key Responsibilities

• Lead and mentor cross-functional teams of nurses, social workers, behavioral health professionals, and care management support associates.
• Direct the day-to-day care management of Medicaid members served in Humana's Illinois market.
• Apply established guidelines and departmental procedures to maintain consistent, high-quality member care.
• Use advanced technical and clinical knowledge to resolve moderately complex challenges and remove barriers to care.
• Promote collaboration across disciplines to deliver coordinated, member-focused outcomes.

Qualifications

• Demonstrated experience leading clinical or care management teams, ideally within a Medicaid or managed care setting.
• Strong working knowledge of care management practices, departmental procedures, and applicable guidelines.
• Proven ability to apply advanced technical expertise to solve moderately complex problems.
• Experience supervising or coordinating nurses, social workers, and behavioral health professionals is highly valued.
• Availability to work on-site full-time in Stateville, Will County (this role is not remote).
