About the Role

As a Home Health Medical Social Worker (MSW) with Frontpoint Health in Dallas, you will travel to patients' homes to assess emotional, social, and environmental factors affecting their care. Working on a PRN basis, you'll collaborate within a multidisciplinary team to coordinate services and connect patients and caregivers with the resources they need.

Key Responsibilities

• Conduct thorough psychosocial assessments for home health patients of all age groups.
• Analyze patient and family circumstances to identify needs and barriers to care.
• Provide supportive counseling to patients and their loved ones in the home setting.
• Identify and arrange appropriate community referrals and support services.
• Contribute actively to the coordination of each patient's overall plan of care.

Qualifications

• Master's or doctoral degree in social work from an accredited program.
• Background or interest in delivering home-based or community health services.
• Strong assessment, counseling, and care-coordination skills.
• Ability to work independently while partnering with a broader clinical team.
• Comfortable traveling to patient residences throughout the Dallas, Texas area.
