Wraparound Facilitator

In this full-time, on-site role, you'll coordinate individualized support that helps children with serious emotional needs and their families build stronger, healthier futures.

About the Role

As a Wraparound Facilitator, you will manage an active caseload under the guidance of your Supervisor, delivering coordinated wraparound services to youth placed with The United Methodist Children's Home. The position centers on guiding children experiencing severe emotional disturbance (SED) and their families through a team-based, strengths-driven planning process that addresses each child's unique circumstances.

Key Responsibilities

• Carry and maintain an ongoing caseload of children referred for wraparound support
• Conduct thorough assessments of each child and family to identify needs, strengths, and goals
• Coordinate and facilitate services tailored to youth living with severe emotional disturbance (SED)
• Partner with families, caregivers, and community providers to create and follow individualized service plans
• Work under the direction of the Supervisor to monitor progress and adjust support as needs change

Qualifications

• Background in social work, counseling, human services, psychology, or a closely related field
• Experience working with children and families, ideally those affected by emotional or behavioral challenges
• Strong organizational skills to manage a caseload and document service activity accurately
• Excellent communication and collaboration abilities for working with families and multidisciplinary teams
• A genuine commitment to advocating for the well-being of at-risk youth
