About This Role
As a Social Worker II, you'll provide comprehensive clinical social work services across our University campus, helping patients manage emotional and social challenges that intersect with their medical care. Working daytime hours within an interdisciplinary team, you'll assess needs, coordinate resources, and ensure patients receive holistic support from admission through discharge and beyond.
What You'll Do
- Complete thorough psychosocial assessments to understand patients' circumstances, strengths, and barriers to recovery
- Develop individualized treatment and discharge plans in partnership with patients, families, and the care team
- Provide crisis intervention and short-term therapeutic support to patients experiencing acute distress
- Connect patients with community resources, benefits programs, and ongoing social services to support their transition
- Document case progress and clinical observations in accordance with healthcare compliance standards
- Advocate for patients' rights and wellbeing while partnering with physicians and nurses to address social factors affecting health
- Respond thoughtfully to family concerns and coordinate complex discharge plans
What We're Looking For
- Bachelor's degree in Social Work or closely related discipline
- Active state licensure as a clinical social worker (LCSW or equivalent) or eligibility to obtain licensure
- 2–3 years of direct clinical social work experience, preferably in a hospital or healthcare organization
- Excellent communication and interpersonal skills with ability to work effectively across diverse patient populations
- Strong case management and organizational ability in a high-volume, dynamic healthcare environment
- Familiarity with Memphis-area community resources, social services, and referral systems
- Genuine commitment to patient-centered care and professional growth
