Social Services Director

About the Role

The Social Services Director leads our facility's social work and counseling programs, acting as a key member of the interdisciplinary care team. You'll manage resident admissions, discharge planning, and ongoing support services while fostering strong relationships with families and external community partners. This role requires both strategic oversight and hands-on engagement to maintain compliance with state and federal guidelines governing skilled nursing facilities.

What You'll Do

• Conduct comprehensive psychosocial assessments and develop individualized care plans for residents
• Coordinate discharge planning and post-acute care transitions
• Maintain regular, meaningful communication with residents' families and support networks
• Collaborate with physicians, nurses, and therapists to ensure integrated, person-centered care
• Connect residents to community resources, mental health services, and recreational programs
• Oversee documentation and recordkeeping to ensure regulatory compliance
• Monitor and advocate for each resident's emotional, social, and spiritual needs

What We're Looking For

• Bachelor's degree in Social Work, Counseling, or related field; MSW preferred
• Current licensure as a clinical social worker or licensed counselor (or equivalent credentials for your state)
• Proven experience directing social services or counseling programs in a healthcare setting
• Expertise in care coordination, discharge planning, and family engagement
• Strong knowledge of long-term care regulations and compliance requirements
• Exceptional interpersonal and communication skills across all levels of staff and residents
