Phoenix Home Care and Hospice is seeking a full-time RN (Registered Nurse) Home Health Case Manager in St. Louis, Missouri, to provide direct clinical oversight and care coordination for patients receiving home-based services. About This Position This is a full-time clinical nursing position based in St. Louis, Missouri. As an RN Home Health Case Manager at Phoenix Home Care and Hospice, you'll combine direct patient assessment with care coordination to optimize outcomes for individuals receiving home health services. You'll work autonomously while maintaining close collaboration with our clinical team, physicians, and specialists to ensure patients' medical, psychosocial, and functional needs are met throughout their care journey.
