About the Role As an RN Home Health Admissions specialist at Phoenix Home Care and Hospice, you'll conduct comprehensive initial patient evaluations and develop the clinical framework for individualized home health services in Kansas City. You'll work closely with physicians, families, and our interdisciplinary care team to ensure smooth transitions into home-based care and establish clear expectations for each patient's treatment plan. This position combines advanced clinical expertise with strong communication skills—you'll help patients and families understand their care options during a critical time.
