About the Role

As an MDS nurse you will serve as the clinical expert responsible for completing the Minimum Data Set for each resident, ensuring that all information meets federal and state requirements while supporting the interdisciplinary team in delivering high‑quality, person‑centered care.

What You'll Do

  • Conduct comprehensive MDS assessments within required timeframes and submit data to the appropriate reporting systems.
  • Collaborate with physicians, therapists, social workers, and direct care staff to develop and update individualized care plans based on assessment findings.
  • Monitor resident outcomes, identify trends, and recommend adjustments to care strategies to promote optimal health and quality of life.
  • Provide education and guidance to nursing staff on MDS processes, documentation standards, and regulatory updates.
  • Maintain accurate, confidential records and participate in internal audits to ensure ongoing compliance.
  • Act as a resource for families and caregivers, explaining assessment results and care plan objectives in clear, understandable terms.

What We're Looking For

  • Current, unrestricted Registered Nurse license in the state of Minnesota.
  • Proven experience performing MDS assessments in a long‑term care or skilled nursing setting.
  • Strong knowledge of CMS regulations, RAI manual, and quality reporting requirements.
  • Excellent organizational skills with the ability to manage multiple assessments and deadlines.
  • Effective communication and interpersonal abilities for working with residents, families, and the care team.
  • Commitment to compassionate, resident‑focused care aligned with a Christian‑based mission.
  • Basic proficiency with electronic health record systems and standard office software.
