Position Summary

As an RN Health Care Facility Surveyor, you will conduct comprehensive evaluations of long-term care facilities to ensure compliance with Centers for Medicare & Medicaid Services (CMS) standards. Your expertise will directly impact the quality of care residents receive, while supporting facility leadership in achieving operational excellence and regulatory adherence across Wisconsin.

Key Responsibilities

• Performing detailed on-site surveys and assessments of long-term care facilities
• Evaluating facility compliance with federal and state long-term care regulations
• Reviewing care practices, staffing levels, documentation, and facility conditions
• Documenting survey findings and preparing comprehensive reports with actionable recommendations
• Collaborating with facility administrators on corrective action plans and follow-up assessments
• Maintaining accurate records and communicating survey results clearly to stakeholders

What You Bring

• Active Registered Nurse (RN) license in Wisconsin
• CMS Long-Term Care Basic Training certification
• SMQT (Surveyor Management and Quality Training) certification
• Working knowledge of Medicare, Medicaid, and long-term care regulations
• Strong professional judgment, attention to detail, and integrity
• Ability to work independently and manage concurrent facility surveys
