This position offers flexible Per-Diem or Full-Time options and a per-visit pay structure: $146 per Start of Care (SOC), $101 per Evaluation, and $91 per Routine visit. Key Responsibilities Conduct in-home physical therapy evaluations and start-of-care assessments for Home Health patients. Design and carry out individualized treatment plans that support recovery, mobility, and independence. Provide routine therapy visits across the Lompoc and greater Santa Barbara County service area. Track patient progress, adjust care plans as needed, and maintain accurate clinical documentation. Work alongside the broader care team to support safe, positive outcomes at home. Qualifications Licensed Physical Therapist (PT) eligible to practice in California. Comfortable working independently in a home health setting. Reliable transportation to travel between patient homes in the Lompoc area. A genuine commitment to compassionate, patient-centered care.
