Physical Therapist – PRN – Skilled Nursing Facility

About the Role

As a PRN Physical Therapist at our skilled nursing facility, you will provide individualized therapy to residents recovering from illness, injury, or surgery. This as-needed position offers scheduling flexibility while allowing you to make a direct impact on each resident's mobility, strength, and independence as part of a collaborative rehabilitation team in Horn Creek, Custer County.

Key Responsibilities

• Assess residents and develop personalized physical therapy treatment plans aligned with their recovery goals.
• Deliver skilled, evidence-based therapy interventions to restore movement, function, and quality of life.
• Track resident progress and adjust treatment approaches as recovery advances.
• Maintain accurate, timely clinical documentation in line with facility and regulatory standards.
• Partner with nurses, occupational and speech therapists, physicians, and families to coordinate well-rounded care.
• Educate residents and caregivers on safe techniques, home programs, and ongoing wellness.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Active Physical Therapist (PT) license valid in the applicable state.
• Experience in skilled nursing, rehabilitation, or geriatric settings preferred.
• Compassionate, patient-centered approach with strong communication skills.
• Availability to work on a PRN (as-needed) basis with flexible scheduling.
