Core Duties Travel to client residences within assigned geographic territories to deliver physical therapy services. Complete thorough evaluations measuring joint mobility and range, transfer abilities, stability, walking patterns and appearance, muscle strength and endurance, balance, and home safety risks. Apply evidence-based integumentary protections and topical agents that shield skin integrity and accelerate wound recovery. Design and execute personalized treatment plans featuring hands-on therapeutic exercise, gait practice, balance work, and related interventions. Reassess and update the physical therapy plan of care continuously while collaborating with nursing colleagues on the broader multi-disciplinary plan. Teach clients, family members, and other caregivers the rehabilitative techniques and daily activities that foster health, safety, and greater independence. Document observations, interventions, and outcomes accurately on a modern touch-pad tablet so every service remains clearly recorded. Who We're Looking For Active, unrestricted Physical Therapist license issued by the Commonwealth of Virginia. Graduation from a physical therapy curriculum approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, verified by transcript or diploma. Proven ability to work independently while organizing visits and managing time effectively across a busy caseload. Strong interpersonal, organizational, and problem-solving capabilities that support complex home-care situations.
