Paradigm is hiring a dedicated Nurse Case Manager in Twin Falls, ID to support injured individuals across Twin Falls and the surrounding Idaho communities. This is a full-time, home-based field role based out of Blackfoot, Bingham County, with regular local travel.
About the Role
As a Field Nurse Case Manager with Paradigm, you will serve as the primary clinical advocate for people recovering from injury, coordinating their care from the field rather than a fixed office. The position is home-based and involves traveling throughout Twin Falls, ID and nearby areas to meet patients, providers, and care teams where they are.
Key Responsibilities
- Manage a caseload of injured patients across Twin Falls and the surrounding region, guiding each through their recovery journey
- Conduct in-person visits with patients, treating physicians, and other members of the care team
- Coordinate treatment plans, appointments, and services to promote timely, appropriate recovery outcomes
- Document case progress accurately and communicate updates to relevant stakeholders
- Travel regularly within the Twin Falls, Idaho area as a core part of this field-based role
Qualifications
- Active registered nursing credentials and clinical experience suited to case management
- Willingness to work from home and travel routinely throughout the Twin Falls and surrounding Idaho areas
- Strong organizational, communication, and care-coordination skills
- Ability to work independently while collaborating with a broader care team
Schedule & Compensation
This role is structured at 28 hours per week, with an annual salary range of $62,520 to $72,800. Should the position begin at 40 hours per week, or move to 40 hours in the future, compensation will be increased proportionally to reflect the additional hours.
