About the Role

This Full-Time LPN/LVN position focuses on delivering direct patient care in our rehabilitation setting. You'll work alongside physicians, therapists, and fellow nursing staff to support patients through their recovery and restoration journey. What You'll Do

• Monitor and document patient vital signs, symptoms, and clinical progress
• Assist patients with activities of daily living and mobility support
• Provide wound care, dressing changes, and specialized patient procedures
• Support catheter care and other medical device management
• Maintain accurate and timely medical records and patient documentation
• Administer nursing care procedures following clinical protocols and physician orders
• Collaborate with the interdisciplinary healthcare team to optimize patient outcomes
• Communicate effectively with patients and families about their care plans

What We're Looking For

• Valid LPN or LVN license (Kentucky or reciprocal state license)
• High school diploma or GED
• Demonstrated clinical assessment and nursing care skills
• Strong communication and interpersonal abilities
• Commitment to compassionate, patient-centered care delivery
• Ability to thrive in a collaborative, dynamic healthcare setting
• Attention to detail and excellent documentation practices
