Housing Stability Case Manager – Rapid Rehousing

Community of Hope, a Washington Post Top Workplace, is hiring a full-time Housing Stability Case Manager – Rapid Rehousing in Forest Heights, Prince George's County to help families and individuals move from crisis into stable, lasting housing.

About the Role

As a Housing Stability Case Manager with the Rapid Rehousing Program, you will walk alongside participants as they secure and maintain permanent housing, connecting them to the healthcare, housing, and community support services they need to thrive. Community of Hope is currently filling four of these positions, each offering an hourly rate of $25.96–$26.92. This is a full-time opportunity for someone who believes everyone deserves a safe place to call home.

Key Responsibilities

• Provide person-centered case management to households enrolled in the Rapid Rehousing Program.
• Support clients through housing searches, lease-up, and the transition into stable, permanent housing.
• Develop individualized plans that address each participant's goals around income, wellness, and self-sufficiency.
• Link families and individuals to healthcare, benefits, and community resources.
• Maintain accurate, timely records and documentation in line with program requirements.
• Partner with landlords, service providers, and colleagues to remove barriers to housing stability.

• Strong interpersonal, organizational, and communication skills.
• Ability to build trusting relationships with clients from diverse backgrounds.
• Comfort with documentation and coordinating across multiple service partners.
