About the Role
As a Host Home Provider with Sample Supports in Larimer County, you will welcome a woman in her 30s with intellectual disabilities along with medical and mental health needs into your home. Rather than placing individuals in institutional settings, this position centers on shared living, so the person you support can remain rooted in her own community while receiving consistent, personalized care.
Key Responsibilities
- Provide a safe, welcoming, and stable in-home placement tailored to the individual's needs.
- Support daily living activities and encourage personal independence and community participation.
- Assist with coordinating and attending to medical and mental health requirements.
- Build a respectful, trusting relationship that promotes dignity, choice, and overall well-being.
- Communicate regularly with the Sample Supports team and care providers.
