About the Role

As the Hospital Liaison for Serenity Estates of Lincolnshire, you will serve as the trusted point of connection whenever a resident moves between our assisted living facility and the hospital. This on-site role in Lincolnshire, Lake County calls for warmth, organization, and a genuine dedication to resident well-being. Key Responsibilities

• Coordinate and oversee resident transfers to and from hospital settings to ensure continuity of care.
• Act as the primary liaison between our assisted living team, hospital staff, residents, and family members.
• Maintain clear, accurate records and communicate critical information during each transition.
• Support residents and families with compassion, guidance, and reassurance throughout the process.
• Partner with clinical and care teams to anticipate needs and deliver seamless, well-organized care.
