Hospice Medical Social Worker (Weekend)

Northern Illinois Hospice Association is hiring a part-time Hospice Medical Social Worker (Weekend) in Morristown, Winnebago County. This weekend position offers you the opportunity to provide compassionate, dignified end-of-life care to patients and families during their most meaningful moments.

About the Role

As a Hospice Medical Social Worker, you'll serve as a vital member of our interdisciplinary care team, helping patients and families navigate the emotional, practical, and social dimensions of their hospice journey. This part-time weekend role focuses on building supportive relationships and connecting people with the resources they need during this important time.

What You'll Do

• Conduct comprehensive psychosocial assessments and develop individualized care plans in partnership with patients and families
• Provide direct counseling and emotional support to patients facing end-of-life concerns
• Support family members through the dying process, helping them communicate, find meaning, and prepare for loss
• Coordinate bereavement resources and refer families to grief support services
• Connect patients and families with community resources, social services, and financial assistance programs
• Collaborate with nurses, physicians, chaplains, and other care team members to ensure holistic support
• Document assessments, interventions, and progress notes in patient medical records
• Help facilitate conversations about values, goals, and end-of-life preferences

What We're Looking For

• Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW)
• Experience in healthcare, hospice, palliative care, or related clinical settings
• Strong communication and active listening skills
• Ability to work independently and collaboratively with interdisciplinary care teams
• Comfort engaging with patients and families across varying cultural, spiritual, and socioeconomic backgrounds
• Compassion, emotional resilience, and deep commitment to honoring patient dignity
