About This Position

Encompass Health Rehabilitation Hospital of Concordville is seeking a dedicated Dialysis RN to provide comprehensive care to patients undergoing hemodialysis treatment as part of their rehabilitation journey. In this full-time nursing role, you'll work alongside a collaborative multidisciplinary team to deliver clinical excellence, patient education, and compassionate support within a rehabilitation-focused environment. This position offers the opportunity to specialize in dialysis nursing while making meaningful contributions to patient recovery and quality of life in our Pottstown facility.

Core Duties

• Monitor and document patient vital signs, lab values, and overall clinical status throughout dialysis sessions
• Operate and troubleshoot hemodialysis equipment, ensuring all devices function safely and effectively
• Administer medications and manage vascular access sites with precision and adherence to protocol
• Assess patient responses to treatment and communicate changes in condition to the care team
• Educate patients about their dialysis treatment, medications, and self-care management
• Maintain accurate and timely nursing documentation in accordance with facility and regulatory standards
• Collaborate with physicians, social workers, dietitians, and other clinical staff to optimize patient outcomes

What We're Looking For

• Current, valid RN licensure in Pennsylvania
• Knowledge of hemodialysis procedures and patient care protocols
• Ability to assess patients accurately and respond promptly to clinical changes
• Strong communication and patient education skills
• Detail-oriented approach to equipment operation and clinical documentation
• Compassion, empathy, and commitment to patient-centered care
• Ability to work effectively as part of an interdisciplinary healthcare team
