Community Liaison

Catholic Hospice seeks a full-time Community Liaison to serve in our Hialeah Lakes office in Miami-Dade County. In this position, you'll be the frontline connection between our community and our hospice services, fielding inquiries, coordinating referrals, and helping guide families toward the compassionate care they need.

About the Role

As Community Liaison, you'll work Monday through Friday, 8:00 a.m. to 5:00 p.m., managing the initial touchpoints for individuals and families seeking information about Catholic Hospice's services. Your role bridges our admissions process with the broader healthcare community, ensuring that every inquiry is handled with care and professionalism.

What You'll Do

• Respond to and address inquiries from community members, referral sources, and healthcare providers seeking information about hospice care and Catholic Hospice's service offerings
• Coordinate scheduling for admissions evaluations, working directly with prospective patients and families
• Maintain communication between our marketing department, admissions team, interdisciplinary clinical staff, and external referral partners to ensure seamless referral processing
• Serve as a knowledgeable resource about Catholic Hospice's programs, eligibility criteria, and service areas within central Miami-Dade County
• Document inquiries and referrals accurately to support administrative and clinical operations

What We're Looking For

• Professional communication skills with the ability to listen with empathy and convey information clearly to diverse audiences
• Organizational ability to manage multiple scheduling requests and referral processes simultaneously
• Customer service experience in healthcare, social services, or a similar field preferred
• Comfort with office technology, including phone systems and scheduling software
• Understanding of or willingness to learn about hospice care and end-of-life services
• Bilingual capabilities (Spanish and English) are a strong asset in serving our Miami-Dade community
