About the Role
As a Clinical Rehab Liaison, you serve as a bridge between our rehabilitation facility and the healthcare providers, facilities, and community partners who refer patients to us. You'll leverage your clinical background to educate stakeholders about our services while managing a dynamic territory and supporting patients' rehabilitation journeys.
What You'll Do
- Build and maintain strong referral partnerships with physicians, case managers, and healthcare facilities throughout Butler and Taylor County
- Conduct thorough patient assessments to evaluate rehabilitation needs and determine appropriate level of care
- Manage your assigned territory strategically, identifying growth opportunities and ensuring consistent stakeholder engagement
- Educate healthcare partners about rehabilitation programs and clinical services
- Coordinate with admissions to facilitate patient placement and gather clinical information
- Communicate patient progress and clinical outcomes to referring providers and facilities
- Support discharge planning and continuity of care initiatives
What We're Looking For
- Current clinical license (required) in your healthcare discipline
- Professional experience in healthcare, rehabilitation, nursing, physical therapy, occupational therapy, or related clinical field
- Excellent interpersonal and communication skills with diverse healthcare professionals
- Proven ability to build trust and cultivate long-term professional relationships
- Comfortable traveling within your service territory
- Strong organizational skills and attention to clinical detail
- Genuine commitment to patient-centered, quality care
