About the Role Working within a permanent supportive housing framework, you'll guide clients across all stages of their housing journey—from street situations through stable permanent placement. Connect clients to community resources, services, and benefits aligned with their goals, providing ongoing navigation support. Administer clinical assessments (including VI-SPDAT) and document findings to guide care planning.
