Join Rincon Family Services as a Case Manager for our Transitional Living Program (TLP) in West Garfield Park, Chicago. This full-time position offers the opportunity to guide young people toward independence and stability while working alongside a dedicated team committed to restoring community strength on Chicago's West Side.

About the Role

Reporting to the Supervisor of Case Management, you will serve as a primary support person for program participants in our TLP and Independent Living Option (ILO) initiatives. This salaried, exempt position combines direct client support with administrative responsibility, requiring both compassion and organizational precision.

What You'll Do

  • Develop and implement individualized case plans that identify participant goals and measurable steps toward independent living
  • Provide regular one-on-one support, coaching, and advocacy for young people navigating housing, employment, education, and life skills challenges
  • Connect participants with community resources including mental health services, job training, educational programs, and financial assistance
  • Monitor progress, track outcomes, and adjust case plans as needs evolve
  • Maintain accurate, timely documentation and case notes in accordance with program standards
  • Collaborate with schools, employers, social services agencies, and family members to support participant success
  • Participate in team meetings and contribute to program improvement initiatives

What We're Looking For

  • Bachelor's degree in social work, counseling, human services, or related field, or equivalent lived or professional experience
  • Demonstrated commitment to supporting young adults facing housing instability or foster care transition
  • Strong listening skills, cultural humility, and ability to build trust across differences
  • Excellent written and verbal communication abilities
  • Reliable transportation and willingness to travel within the Chicago area
  • Experience with case management systems and databases is a plus
