This is a full-time, on-site Case Manager RN opportunity in Appleton, Outagamie County, structured as a 13-week travel assignment with the potential for extension. The position follows a five-day schedule of eight-hour day shifts (8:00 AM to 4:30 PM), totaling 40 hours each week. Compensation for qualified travelers reaches $1,900 to $2,600 in gross weekly pay, supplemented by an additional $500 completion bonus.

Key Responsibilities

• Coordinate and oversee individualized care plans for patients across the continuum of care.
• Evaluate patient needs and connect them with appropriate clinical and community resources.
• Collaborate with physicians, nurses, and the broader interdisciplinary care team to support positive outcomes.
• Track patient progress and adjust care planning to align with evolving treatment goals.
• Maintain accurate, timely documentation in accordance with facility and regulatory standards.

Qualifications

• Active, unencumbered Registered Nurse (RN) license valid for practice in Wisconsin.
