About the Role

As a Case Manager I with Interfaith Community Services, you will serve as a frontline advocate and resource navigator for individuals and families experiencing hunger, homelessness, and economic hardship. This on-site position in Escondido connects vulnerable clients with the programs and support they need to build stability and independence.

Key Responsibilities

  • Conduct intake assessments to understand each client's needs, strengths, and goals.
  • Develop individualized service plans that map a realistic pathway toward self-sufficiency.
  • Connect clients to internal programs and external community resources, including housing, food, and supportive services.
  • Track client progress, document case activity, and maintain accurate, confidential records.
  • Collaborate with program staff and partner agencies to coordinate wraparound support.
  • Provide encouragement and follow-up that empowers clients to help themselves.

Qualifications

  • A genuine commitment to serving low-income, homeless, and at-risk populations with dignity and respect.
  • Strong interpersonal and communication skills for building trust with diverse clients.
  • Organizational ability to manage a caseload, documentation, and competing priorities.
  • Comfort working collaboratively within a mission-driven social services team.
  • Availability for a full-time, on-site schedule in Escondido, San Diego County.
