Tracy Community Connections Center is hiring a full-time Case Manager to support individuals and families seeking stable housing and sustainable tenancy in Tracy, California. In this role, you'll provide direct case management services to help clients navigate the housing landscape, maintain their tenancy, and build long-term housing stability.
About the Role
As a Case Manager for the Housing Tenancy & Sustainability Program, you'll be the primary support person for each client, guiding them through intake, assessment, planning, and ongoing service coordination. Working under the Director of Case Management, you'll deliver person-centered support aligned with state health care guidelines, connecting clients with resources and monitoring progress toward their housing goals.
What You'll Do
- Conduct comprehensive intake assessments to understand each client's housing situation, barriers, and strengths
- Develop and implement individualized service plans that address housing stability and sustainability needs
- Coordinate referrals and support services with community partners, agencies, and landlord resources
- Track client progress, document outcomes, and maintain accurate, timely case notes and records
- Connect clients with emergency assistance, rental support, utility assistance, and other resources
- Maintain regular contact through in-person visits, phone calls, or virtual check-ins
- Ensure compliance with program requirements and regulatory standards
What We're Looking For
- High school diploma or equivalent; associate's degree in social services, counseling, or related field preferred
- Experience in case management, social services, or community health support
- Strong communication and problem-solving abilities
- Ability to work compassionately with clients facing housing instability and poverty
- Proficiency with data entry and case management software
- Valid California driver's license and reliable transportation
- Willingness to build trusted relationships across diverse communities
