About This Position
We are seeking a dedicated Case Manager to join our Port Chicago team and play a vital role in clients' lives. As a case manager at TASC, you will serve as both advocate and coordinator, connecting clients with resources, monitoring their progress, and helping them navigate the systems and services that support their recovery journey. This is full-time, on-site work in Port Chicago where you'll be part of a team committed to breaking barriers and reshaping lives.
Responsibilities
- Conduct thorough intake assessments and evaluations to understand each client's needs, strengths, and barriers to recovery
- Develop individualized service plans in partnership with clients, incorporating their goals and input into every step
- Connect clients with and coordinate access to treatment services, housing support, employment programs, education opportunities, and community resources
- Track client progress, adjust service plans as circumstances change, and maintain clear communication across the care team
- Advocate effectively on behalf of clients when engaging with social service systems, treatment providers, and community partners
- Document all client interactions, progress, and plan modifications accurately to ensure compliance and continuity of care
- Participate in team meetings and contribute to a collaborative, supportive work environment
What We're Looking For
- High school diploma, GED, or equivalent; associate's or bachelor's degree preferred
- Genuine passion for supporting individuals facing barriers to recovery and community integration
- Strong organizational, written, and verbal communication skills
- Ability to manage multiple cases and priorities while remaining detail-oriented
- Comfort working both independently and collaboratively within a team setting
- Reliable transportation for on-site work in Port Chicago
- Knowledge of social service systems, community resources, or case management best practices is valuable
