Foothill Family is hiring a full-time Case Manager, Adult Protective Services in Bradbury, Los Angeles County, to provide compassionate, short-term care coordination for some of our community's most vulnerable adults.
About the Role
As an Adult Protective Services (APS) Case Manager with Foothill Family in Bradbury, you will guide clients facing complex medical, psychosocial, functional, and housing challenges toward greater safety and stability. This client-centered position focuses on reducing risk, connecting individuals to vital services, and supporting housing stabilization—all while honoring each person's autonomy, cultural background, and legal rights.
Key Responsibilities
- Deliver short-term, individualized care coordination to APS clients with overlapping medical, social, and functional needs.
- Assess client situations and develop strategies that minimize risk and promote personal safety.
- Coordinate and link clients to appropriate community resources and supportive services.
- Assist clients in achieving and maintaining stable housing arrangements.
- Respect and uphold each client's right to self-determination, cultural preferences, and legal protections throughout the service process.
Qualifications
- Experience in case management, social work, or care coordination, ideally serving adults with complex needs.
- Working knowledge of psychosocial, medical, and housing-related challenges affecting at-risk adult populations.
- Skill in service navigation, risk assessment, and client advocacy.
- A commitment to client dignity, cultural sensitivity, and ethical, rights-based practice.
- Strong communication and organizational abilities to manage a varied caseload effectively.
