About the Role
As a Care Manager with FreedomCare in New Haven, CT, you will work in the field, meeting patients where they live and helping them navigate home-based care. This position puts you at the heart of our mission: empowering people to receive the support they need in the comfort and familiarity of their own homes. The role is full-time and field based, requiring regular travel throughout the New Haven, New Haven County area.
Key Responsibilities
- Build and maintain trusted relationships with patients and their chosen caregivers across New Haven County.
- Conduct in-person visits to assess patient needs and coordinate appropriate home-based care.
- Guide patients and families through the process of selecting and working with a caregiver.
- Serve as a dependable point of contact, advocating for each patient's well-being and continuity of care.
- Collaborate with internal teams to ensure care plans reflect the values and standards FreedomCare is known for.
Qualifications
- Genuine commitment to patient-centered, in-home care.
- Strong interpersonal and communication skills for working directly with patients and caregivers.
- Ability to work independently in a field-based capacity, traveling throughout the New Haven area.
- Comfort coordinating care details and maintaining organized, reliable follow-through.
