Specialist, IRIS Consulting

Build meaningful, lasting partnerships while supporting people with long-term care needs as a Specialist, IRIS Consulting at Molina Healthcare. This is a full-time, remote role serving participants enrolled in the Wisconsin IRIS Program through the TMG IRIS Consultant Agency.

About the Role

As a remote IRIS Consultant based in your home community, you will guide older adults and people with disabilities through the Wisconsin IRIS program — a self-directed Medicaid long-term care option. Although your workspace is home-based, you will travel for scheduled in-person visits with participants in their homes and communities. Together with each participant, you will set long-term care goals and discover creative paths to reach them. No two days look exactly alike.

Key Responsibilities

• Meet face-to-face with each IRIS participant at least four times annually, including one required visit in the participant's home, and remain responsive to requests in this self-directed program.
• Deliver program orientation covering participant rights, responsibilities, legal document verification, employee paperwork, and the appropriate use of public funds.
• Explore each participant's broader life — goals, relationships, community ties, employment interests, the Self-Directed Personal Care option, and back-up support plans.
• Help identify personal outcomes and track progress while staying within IRIS budgets and Department of Health Services (DHS) requirements.
• Document orientation and planning activities in the IRIS data system (WISITs) within 48 business hours of each visit, and update plans whenever changes are requested.
• Research community resources and natural supports, prepare budget amendment or one-time expense paperwork as needed, and coach participants on reading monthly budget reports.
• Serve as a liaison with the Fiscal Employer Agency on billing, support brokers, receipt tracking, worker payment, and conflict-of-interest concerns.
• Maintain confidentiality and HIPAA compliance, collaborate with agency colleagues, and attend monthly in-person team meetings plus periodic supervisor check-ins.

Qualifications

• Minimum two years in health care, ideally care coordination, plus at least one year serving IRIS target groups (adults with intellectual or physical disabilities or older adults), or an equivalent mix of education and experience.
• Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a related field — or four years of comparable experience.
• Valid, unrestricted driver's license, dependable transportation, and adequate auto insurance for required travel, unless otherwise required by law.
• Talent for building positive relationships with coworkers, participants, providers, and regulatory agencies.
