Key Responsibilities

  • Reviews the content of medical records, including laboratory findings, radiology reports, discharge summaries, history and physicals, consultations, orders, progress notes, and other ancillary services treatment records, to identify principal diagnosis, secondary diagnoses, and procedures performed.
  • Ensures compliance with standard provider coding regulations by carefully detailing reviews of documents needed to ensure all pertinent diagnoses and procedures are recorded.
  • Translates diagnostic and procedural phrases into coded form using procedure codes as required.
  • Determines the appropriate DRG for each patient encounter, ensuring accurate reimbursement through proper sequencing of codes.
