Day-to-Day Responsibilities

• Answer and triage on-call calls or messages from patients, families, and community partners throughout weekend duty periods
• Complete telephone or in-person evaluations to identify changes in symptoms, comfort levels, or care requirements
• Implement or adjust nursing measures for pain relief, symptom control, and overall comfort according to the patient’s plan of care
• Communicate findings promptly with physicians and the interdisciplinary hospice team to update orders and share critical information
• Maintain clear, timely documentation of every contact, assessment, and action taken within the clinical record system
• Provide practical guidance and emotional reassurance to caregivers so they can continue supporting the patient at home or in the community

Who We're Looking For

• Current and unrestricted Registered Nurse license issued by the state of Texas
