About the Role
As an Intake Coordinator, you'll be the first point of contact for new clients exploring home care options. You'll conduct comprehensive client consultations, document essential health and personal information, and connect people with the services that match their needs and goals.
What You'll Do
- Conduct intake interviews and assessments with prospective clients and their families
- Gather and organize detailed medical, health, and personal background information
- Evaluate client needs and goals to recommend appropriate care services
- Record all intake data accurately and securely in accordance with privacy and compliance standards
- Explain service options, care models, and what clients can expect from Infinity Love Home Care LLC
- Schedule follow-up assessments and coordinate with care management and clinical teams
- Manage intake records, documentation, and administrative tasks in an organized manner
- Respond to inquiries and address client concerns via phone, email, and face-to-face meetings
What We're Looking For
- High school diploma or GED equivalent
- Professional communication skills, both written and verbal
- Strong organizational ability and meticulous attention to detail
- Competence with computer systems and standard business software
- Availability to work full-time at our Philadelphia location
- Empathetic, respectful approach to client interactions
- Dependable work ethic and consistent punctuality
- Prior experience in healthcare, social services, or client-facing roles preferred
