About the Role
Recovery Network Of Programs is seeking a dedicated Case Manager to support clients through their recovery journey. Since 1972, our nonprofit has served the Greater Bridgeport and Fairfield County communities with comprehensive programs addressing substance use, mental health, and homelessness. As a Case Manager, you'll be the bridge connecting clients with the resources, support systems, and opportunities they need to rebuild their lives with hope and stability.
What You'll Do
- Develop individualized treatment and recovery plans that address each client's unique circumstances and goals
- Connect clients with housing, employment, healthcare, and social services tailored to their needs
- Provide ongoing assessment, monitoring, and support to ensure clients achieve sustainable recovery outcomes
- Maintain accurate documentation and communicate progress with the treatment team
- Navigate complex systems on behalf of clients to advocate for their access to services and benefits
- Build trust-based relationships that empower clients toward independence and wellbeing
What You Bring
- A deep commitment to supporting individuals in recovery from substance use, mental health disorders, or housing instability
- Strong organizational and communication skills, with the ability to work effectively across disciplines
- Experience in case management, social work, healthcare, or related fields serving vulnerable populations
- Ability to manage multiple priorities, maintain detailed records, and work with confidentiality and integrity
- Compassion, patience, and genuine belief in people's capacity for change
