The Affiliated Sante Group is seeking a Community Development Counselor to join our Rock Creek team in Fairland, Montgomery County. In this full‑time position, you will support individuals with intellectual and developmental disabilities and co‑occurring disorders, helping them achieve greater independence and quality of life.
About the Role
As a Community Development Counselor, you will provide direct support and guidance to clients, fostering skill development and community participation while upholding the organization’s commitment to empowering each person to live a fulfilling life.
What You'll Do
- Conduct individualized counseling and skill‑building sessions tailored to each client’s needs.
- Develop, implement, and regularly review person‑centered service plans.
- Collaborate with families, guardians, and interdisciplinary teams to coordinate care.
- Track client progress, document outcomes, and adjust interventions as required.
- Facilitate community integration activities, vocational training, and recreational outings.
- Advocate for clients’ rights and assist them in accessing essential resources and services.
What We're Looking For
- Bachelor’s degree in human services, social work, psychology, or a related field.
- Proven experience working with individuals with intellectual/developmental disabilities and co‑occurring disorders.
- Strong interpersonal, communication, and active‑listening abilities.
- Skill in conducting assessments and creating measurable, goal‑oriented plans.
- Familiarity with person‑centered approaches and evidence‑based practices.
- Valid driver’s license and reliable transportation for community‑based visits.
- Commitment to ethical practice, cultural sensitivity, and continuous learning.
