About the Role
In this role you will work closely with the Rapid Re‑Housing Services (RRHS) team to help clients secure and maintain stable housing. You will provide direct case management, coordinate housing‑related resources, and contribute to a safe, welcoming environment for program participants.
What You'll Do
- Conduct intake assessments and develop individualized service plans for clients entering permanent supportive housing.
- Provide ongoing case management, including goal setting, progress monitoring, and crisis intervention as needed.
- Facilitate access to housing resources, benefits, and community services that support long‑term stability.
- Maintain accurate client records, ensuring timely documentation and reporting in compliance with funder and agency requirements.
- Participate in team meetings, outreach initiatives, and program coverage shifts to uphold service continuity.
- Collaborate with RRHS colleagues to identify barriers to housing retention and develop practical solutions.
What We're Looking For
- Experience providing case management or direct support to individuals experiencing homelessness, mental health challenges, or substance use concerns.
- Strong interpersonal skills with the ability to build trust and rapport across diverse populations.
- Proficiency with data entry and record‑keeping systems; attention to detail is essential.
- Knowledge of housing first principles, harm reduction approaches, and local social service networks.
- Ability to work both independently and as part of a multidisciplinary team in a fast‑paced setting.
- Valid driver’s license and reliable transportation may be required for certain outreach activities.
