About the Role

As a Hospice Social Worker with Compassus, you'll serve as a vital member of our interdisciplinary care team, supporting patients and their loved ones through the end-of-life journey. This full-time position focuses on addressing the emotional, social, practical, and spiritual needs that arise during hospice care, helping families navigate complex decisions and transitions with dignity and support.

What You'll Do

• Conduct comprehensive psychosocial assessments to identify patient and family needs, concerns, and goals
• Develop and implement individualized care plans that address emotional support, coping strategies, and family dynamics
• Provide grief counseling and bereavement support to patients and families before, during, and after end-of-life care
• Coordinate community resources, social services, and support systems for patients and caregivers
• Facilitate advance care planning discussions and help families navigate healthcare decisions
• Collaborate with nurses, physicians, chaplains, and other team members to ensure holistic, patient-centered care
• Document assessments and interventions in compliance with regulatory and ethical standards
• Support families in addressing practical concerns such as financial resources, housing, and insurance matters

What We're Looking For

• Current state licensure as a Master's-level Social Worker (LMSW or LCSW) in Pennsylvania
• Experience working in hospice, palliative care, healthcare, or related patient-centered environments
• Strong communication and active listening skills with demonstrated ability to support vulnerable populations
• Knowledge of grief, bereavement, and psychosocial dynamics in end-of-life care
• Ability to work effectively within interdisciplinary teams while maintaining professional boundaries
• Commitment to ethical practice and cultural humility in serving diverse communities
