About the Role
As a Care Coordinator, you will serve as a bridge between patients and their healthcare providers, ensuring seamless coordination across all aspects of their care. This position focuses on guiding patients through their care journey, managing their health needs, and facilitating communication among medical teams.
What You'll Do
- Manage and maintain detailed patient caseloads to ensure continuity of care
- Arrange and monitor appointments with primary care physicians and specialists
- Navigate patients through care transitions, including hospital discharges, referrals, and care setting changes
- Work closely with patient scheduling teams to secure timely follow-up appointments and reduce gaps in care
- Facilitate interdisciplinary communication among physicians, nurses, specialists, and other care team members to coordinate comprehensive treatment plans
- Educate patients on their diagnoses, treatment options, and self-management strategies
- Track patient outcomes and adjust care coordination plans as needed
What We're Looking For
- Experience in healthcare coordination, case management, or a related patient-facing role
