Home Care Intake Worker

Old Colony Elder Services is hiring a Full-Time Home Care Intake Worker in Brockton, Plymouth County to join our growing team. As we expand services to 11 additional towns across the South Shore, we're seeking dedicated professionals to serve as the vital first point of contact for older adults and individuals with disabilities seeking support and care coordination.

About the Role

As a Home Care Intake Worker, you'll be the compassionate first point of contact for individuals and families reaching out for elder care and disability services. Your role bridges the gap between those in need and the comprehensive support services that can transform their lives, ensuring they receive proper assessment and appropriate resources to maintain healthy, safe, and independent living.

What You'll Do

• Conduct comprehensive intake assessments with new clients to understand their care needs, health status, living situation, and support systems
• Gather and document essential client information accurately and professionally while maintaining confidentiality and sensitivity
• Explain available services and programs, helping clients understand options that address their specific needs
• Determine eligibility for services based on established criteria and organizational guidelines
• Connect clients with appropriate care coordination, social services, and community resources
• Communicate effectively with healthcare providers, social workers, and internal teams to ensure seamless service transitions
• Maintain detailed records and intake documentation to support care planning and continuity of services
• Respond professionally to inquiries via phone and in-person meetings in a timely manner

What We're Looking For

• High school diploma or GED required; some college or certifications in social services, healthcare, or related fields preferred
• Strong communication skills—ability to listen with empathy, explain options clearly, and build rapport with diverse populations
• Organizational skills and attention to detail for accurate record-keeping and documentation
• Ability to work independently and as part of a collaborative team
• Proficiency with computer systems and electronic documentation (training provided)
• Genuine commitment to supporting older adults and individuals with disabilities
• Reliability and professional demeanor in all client interactions
