About the Role

As a Hospice Social Worker, you'll serve as a vital member of our interdisciplinary care team, addressing the emotional, social, and spiritual needs of our patients and their loved ones. You'll conduct thorough psychosocial assessments, develop individualized care plans, and connect families with resources that enhance quality of life during this significant time. Your expertise will help our patients and families find dignity, peace, and meaningful closure.

What You'll Do

• Conduct comprehensive psychosocial and social history evaluations for new patients and their families
• Provide individual and family counseling to address grief, anxiety, and emotional concerns
• Develop and implement discharge and care coordination plans aligned with patient and family goals
• Facilitate referrals to community resources, financial assistance programs, and spiritual care services
• Collaborate with physicians, nurses, and other interdisciplinary team members to ensure holistic care delivery
• Document patient interactions and progress notes in accordance with regulatory and organizational standards
• Offer bereavement support and grief counseling to families before and after patient death
• Advocate for patient rights and cultural preferences in all care planning decisions

What We're Looking For

• Master's degree in Social Work (MSW) from an accredited university
• Current state licensure as a Licensed Clinical Social Worker (LCSW) or eligibility for Ohio licensure
• Experience in hospice, palliative care, healthcare, or grief counseling preferred
• Strong interpersonal and communication skills with ability to work compassionately with diverse populations
• Proficiency in medical record documentation and electronic health records
• Valid driver's license and reliable transportation for in-home patient visits
