Westmoreland Care & Rehab Center
Westmoreland Care & Rehab Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our clinical care team in Lafayette, Macon County. As a full-time LPN, you'll provide skilled bedside care in our 100-bed facility specializing in short-term rehabilitation, long-term care, and outpatient therapy services.
About the Role
This full-time Licensed Practical Nurse position places you at the heart of patient care. You'll work alongside registered nurses and physicians to monitor patient conditions, deliver hands-on care, and support recovery and wellness goals across our facility.
What You'll Do
- Monitor and record patient vital signs, communicating significant changes to nursing leadership and physicians
- Provide direct patient care including hygiene, comfort measures, and assistance with daily activities
- Support medication administration under the direction of licensed RNs
- Perform wound care and skin assessments, documenting findings
- Maintain accurate and timely patient medical records
- Collaborate with the healthcare team on individualized patient care plans
- Respond attentively to patient needs and concerns
What We're Looking For
- Current, unrestricted Licensed Practical Nurse (LPN) license
- Current CPR/BLS certification
- Prior experience in a skilled nursing facility, hospital, or clinical healthcare setting
- Strong verbal and written communication abilities
- Team-oriented mindset with ability to work collaboratively
- Genuine compassion for patients and commitment to dignified, quality care
- Professional reliability and strong work ethic
