Rochester Center seeks a full-time Registered Nurse to serve as MDS Coordinator at our Rochester, Monroe County facility. In this clinical leadership role, you'll manage comprehensive resident assessments and care planning while ensuring compliance with regulatory standards.

About the Role

As MDS Coordinator at Rochester Center, you'll lead assessment and care planning operations that directly impact resident outcomes. This hands-on position requires both clinical expertise and meticulous attention to documentation. You'll work closely with our interdisciplinary care team to deliver evidence-based, individualized care plans that guide treatment and support across the facility.

What You'll Do

  • Conduct comprehensive Minimum Data Set (MDS) assessments with accuracy and clinical precision
  • Develop, implement, and revise individualized care plans based on resident needs and regulatory requirements
  • Establish and maintain care planning calendars, distributed monthly to all relevant departments and care teams
  • Perform systematic audits of clinical records to verify accuracy, completeness, and timeliness of documentation
  • Update and maintain all resident assessments and care plans in accordance with facility protocols and standards
  • Coordinate care planning activities across nursing, therapy, social services, and other departments
  • Ensure all documentation reflects current clinical status and supports quality resident outcomes

What We're Looking For

  • Current New York State Registered Nurse (RN) license
  • Experience with MDS assessments and long-term care documentation
  • Strong organizational skills and ability to manage multiple care plans simultaneously
  • Meticulous attention to detail and commitment to accurate, timely record-keeping
  • Ability to communicate effectively across clinical disciplines
  • Knowledge of regulatory requirements in long-term care settings
