Case Manager

Encompass Health Rehabilitation Hospital of Amarillo is seeking a dedicated Part Time Case Manager to join our care coordination team in Panhandle, Carson County. About the Role

As a Case Manager at our rehabilitation hospital, you'll serve as a bridge between our clinical teams, patients, and their families—ensuring seamless, person-centered care from admission through discharge and beyond. Your work directly influences patient outcomes and helps each person achieve their maximum potential during recovery.

What You'll Do

• Develop and coordinate comprehensive care plans tailored to each patient's medical needs and goals
• Communicate regularly with the interdisciplinary clinical team to ensure coordinated, efficient treatment
• Serve as the primary contact for patients and family members, addressing concerns and keeping them informed throughout their stay
• Manage insurance authorizations, benefits verification, and payer communications
• Plan and facilitate safe, appropriate discharge placements with follow-up support
• Document patient progress, care decisions, and coordination activities in the medical record
• Advocate for patients' needs and preferences within the care team

What We're Looking For

• Current licensure as an RN, LSW, or related healthcare professional, depending on your state and facility requirements
• Experience in case management, discharge planning, or healthcare coordination
• Strong communication and interpersonal skills with patients, families, and medical teams
• Ability to manage multiple priorities and work collaboratively in a fast-paced setting
• Proficiency with electronic health record systems and basic computer skills
• Genuine commitment to patient advocacy and quality outcomes
