BoldAge PACE in Keasbey, Woodbridge is seeking an experienced Care Team Manager with a current Registered Nurse license to lead clinical operations and deliver exceptional senior care in a full-time position. About the Role
As Care Team Manager at BoldAge PACE, you'll oversee the clinical and operational performance of our care team, ensuring every participant receives coordinated, compassionate care. You'll combine leadership, clinical expertise, and strategic thinking to build a high-performing team that puts our seniors first.
What You'll Do
- Supervise and mentor nursing and care staff, fostering a culture of professional excellence and accountability
- Conduct clinical assessments and coordinate individualized care plans for PACE participants
- Manage team scheduling, staffing needs, and workflow optimization to meet program demands
- Monitor quality metrics, compliance with regulations, and adherence to care protocols
- Communicate with participants, families, interdisciplinary team members, and external providers
- Support staff development through training, feedback, and career growth opportunities
- Contribute to continuous improvement initiatives and operational efficiency
What We're Looking For
- Current, active Registered Nurse (RN) license in New Jersey
- Nursing experience and demonstrated leadership in healthcare settings
- Knowledge of PACE program operations or senior care strongly preferred
- Strong clinical judgment and ability to make sound decisions in complex situations
- Excellent communication skills and ability to build relationships across departments
- Commitment to person-centered care and improving quality of life for seniors
