About the Role
As a dialysis social worker at Avera St Luke's Hospital, you'll be a vital member of our interdisciplinary renal care team. Your focus is on comprehensive psychosocial support for dialysis patients—addressing the emotional, social, and practical challenges of chronic kidney disease and long-term treatment.
What You'll Do
- Provide counseling and psychosocial assessment to dialysis patients and their families
- Develop and implement individualized care plans addressing social, emotional, and functional needs
- Connect patients with community resources, support services, and financial assistance programs
- Collaborate with nephrologists, nurses, and clinical staff to ensure holistic patient care
- Document patient interactions and progress in accordance with HIPAA and facility standards
- Support patient education and adherence to treatment plans
- Advocate for patient rights and dignity within the healthcare setting
What We're Looking For
- Master's degree in Social Work (MSW) from an accredited program
- Current LCSW licensure or eligibility for licensure in South Dakota
- Experience in medical social work, renal care, or dialysis settings (preferred)
- Strong communication and interpersonal skills
- Ability to work compassionately with patients facing chronic illness
- Familiarity with insurance, Medicare, and Medicaid regulations
- Proficiency with electronic health records (EHR)
